Job Description

A Sussex based Housing Association is seeking a part time Floating Support Worker to play a key role within the Floating Support Team.

You will work a minimum of 18.5 hours a week Monday – Friday however you could work up to 23 hours a week if required. You will be responsible for supporting a range of client groups housed within the community including clients with mental health issues, young people and learning disabilities. You will assist in the process of resettlement and move-on from supported housing into other options. You will meet with your clients and provide regular support plans, review and assess needs through key working to enable them to gain greater independence and combat social exclusion. You will also be liaising with external agencies, ensuring clients receive assistance with housing benefit claims, community care grants, and other services required.

To be considered for this role you should have extensive experience working with vulnerable client groups and have experience of working within the Supporting People framework.
