Job Description

A Housing Association is seeking a Floating Support Worker to help vulnerable people with support needs.

These individuals possess low-level mental health, homelessness and drug or alcohol problems. You will support these people, who are currently housed in self-contained accommodation, to improve their quality of life and sustain their tenancies. You will meet with your clients for support planning and review sessions, helping them to identify issues and solutions to promote greater living independence.

You will assess and refer clients to key worker/services which best suits their needs. In this capacity you will be in regular contact with external and internal agencies to maximise client support services and resources.

To be considered for this role you must have experience of working with vulnerable client groups, including clients with mental health issues. Experience of assessment and planned support/key working is also important. A current CRB Disclosure is required. A driving license or car user is desirable however not essential.
